UNC Rex — part of the UNC Health system — dramatically expanded its cardiac rehabilitation footprint across North Carolina by partnering with Aviary Health to offer a virtual care option to patients who would otherwise go without.

Before launching Aviary’s virtual cardiac rehab program in the Summer of 2023, UNC Rex’s reach was effectively limited to patients who could make the trip to its Raleigh and Cary locations. Today, UNC Rex is serving patients across 150 unique zip codes — many of them 2 to 4 hours away — delivering over 643,000 minutes of virtual care and growing CR session volumes by 2.8x, all with a zero-day waitlist for virtual care. For a national patient population where only 1 in 4 eligible heart failure patients are ever referred to cardiac rehab and just 1 in 5 complete the full 36 sessions, that kind of reach is transformational. Contact Us to learn more about how Aviary Health partners with health systems to expand their geographical reach without building new brick-and-mortar facilities.
